- STPM/A Level/Foundation in Health Science/ or equivalent
- Minimum CGPA of 2.5 in Biology and Chemistry AND credit in Biology, Physics, Mathematics, Chemistry and English
- Diploma Related Diploma from recognized institution with minimum CGPA of 2.75
- Others Related Diploma from recognized institution with CGPA below than 2.75 and minimum working experience of 3 years in related field
